Beta Launch – Transforming Multi-Morbidity Care

We are excited to officially launch the Beta version of Appt Health’s new multi-morbidity service, a groundbreaking initiative designed to transform the way primary care supports patients with multiple long-term conditions.

At Appt Health, we have always been committed to making preventive healthcare more accessible. Many of you may know us for our recall work, where we have successfully improved patient engagement for screenings and routine care. Now, with the support of Innovate UK funding, we are expanding our focus to address the pressing challenges of multi-morbidity care with an innovative, AI-powered approach.

Why we built this

Our journey began with our founder, Hector Smethurst, who, while working in an East London GP practice, witnessed firsthand the difficulties patients with multiple conditions face in accessing coordinated care. Healthcare systems and the technology that supports them have traditionally been structured around single-condition models, often leaving multi-morbidity patients underserved and practices struggling with complex care management.

Over the past nine months, we have collaborated closely with primary care networks (PCNs) and practice staff to co-design a solution that directly addresses these challenges. Through extensive research and engagement, we identified key barriers to delivering effective multi-morbidity care, including:

-Complex patient searches that make it difficult to identify and prioritizse patients who need care the most.

-Balancing skill mix and clinical capacity, ensuring that patients see the most appropriate healthcare professional at the right time.

-Engaging underserved patient groups, many of whom are frequently missed by traditional recall systems.

With these insights, we developed a comprehensive, data-driven solution to help GP practices streamline multi-morbidity management, reduce administrative burdens, and improve patient outcomes.

What our new multi-morbidity service does

Our service is designed to support GP practices in delivering more efficient, coordinated care for patients with multiple conditions. Key features include:

  • Smart Patient Identification – Automatically identifies patients with existing diagnoses who have overdue care needs.
  • Intelligent Scheduling – Prioritises patients based on clinical need while maximising appointment capacity.
  • Automated Engagement – Sends accessible, behaviourally informed messages at the right time, ensuring better patient response rates.
  • Seamless NHS Integration – Works directly with EMIS, automating administrative tasks to improve efficiency and reduce workload.

At Beta launch, the service currently supports patients with:

1.Hypertension

2.Type 2 Diabetes

3. Chronic Kidney Disease (CKD)

4.Chronic Obstructive Pulmonary Disease (COPD)

We are continuously evolving, and Asthma and Non-Diabetic Hyperglycaemia are next on our roadmap, with further expansions planned.

We are offering a free Beta phase to a limited number of GP practices, allowing early adopters to play a key role in shaping this service. Primary care teams in East London are already experiencing the benefits, and we welcome more practices to join us in transforming multi-morbidity care.
